To make application for the Silver Rose Award
for yourself, a fellow Vietnam Veteran (living or deceased)
or a Vietnam Veteran family member (living or deceased)
first determine that a qualifying disease
is involved and then

When you are on that page print an Application. 
Complete it exactly as instructed
and mail it to the National Director at
the address shown on the Application.
